Also Octopus: you need to fill out this PDF. You need to select which options apply to you (oh, the PDF is not a form. You can't actually select anything - hope you have a PDF mark-up tool! Or why not print it and scan it?!), and add your account number and address (there is no area for this, so just put it in some random whitespace in the PDF and hope that that is what we mean). Yes, this just duplicates everything that you have already sent us by email.
