Perhaps the biggest long term implication is no more Supreme Court picks for Trump. If Dems win the presidency in 2028 we pick up Alito and Thomas's seats most likely, maybe even Roberts.

I read an article that contested this. The right leaning justice who is expected to resign soon does so immediately than it is possible for the existing Republican Senate with the President could get a replacement approved before the new Senate takes over in January.
That fixes a right SCOTUS probably past a future Democratic administration.
Otherwise if he doesn't last the next two years the President & the Senate will veto each other as under Obama weakening and shortening the right SCOTUS majority.
A big decision for the GOP.
