@TerryHancock @CStamp in Britain you are encouraged to sign it when you get your first provisional driving licence (learners permit)
Until recently NHS UK Organ Donation network used VHF pagers for their real time messaging, and with certain equipment you could see what was on offer to the hospitals.
I've often referred to this process as "Copart for humans" (especially when encouraging safe driving)

I think so. It's just a checkbox on the driver's license form here, I believe.
